What are Cataracts?
But what exactly are cataracts?
Cataracts are a clouding of the lens within the eye, which can slowly diminish one’s vision. This clouding is caused by a buildup or protein within the lens, and it can become increasingly worse over time. In a healthy eye, the lens is clear and allows light to pass through easily to reach the retina — the light-sensitive tissue located at the back of the eye. Eyes with cataracts, on the other hand, have a cloudy lens which results in blurred vision.
There is no one factor that causes the development of cataracts. However, many eye doctors believe that certain habits, such as smoking, may put a person at risk for developing this condition. Additionally, health conditions like diabetes and overexposure to ultraviolet light have also been linked to the development of cataracts.
Some patients who develop cataracts may experience no symptoms of the condition at all, while many experience blurred or double vision. Other possible signs include frequent prescription changes in contact lenses or eyeglasses, as well as a diminished ability to see clearly at night.
For most patients, treating cataracts will require surgery. Cataract surgery is one of the most commonly performed procedures in the country today, and offers patients extremely effective results. It is a minimally-invasive procedure, with the main goal of removing the damaged lens from the eye. The damaged lens will then be replaced with an IOL (artificial intraocular lens).
Following this procedure, patients may experience some mild discomfort and itchiness. For several days after, eyes may also be extremely sensitive to light. Fortunately, any discomfort is usually mild and typically lasts only a few days. Patients who undergo this surgery will need to protect their eyes afterwards, and will have to avoid rubbing their eye as they recover.

Study Shows ORA Can Improve Toric Lens Outcomes
Eye Consultants of Texas was the first Dallas/Fort Worth practice to use what is now the ORA System – the first intraoperative abberometer technology that provides real-time measurements of the eye during cataract surgery. On January 1, 2014, an Ophthalmology Times online article appeared about a study which showed ORA technology significantly improves refractive and visual outcomes after toric intraocular lens (IOL) implantation.
According to the article, “Intraoperative Aberrometry Adds Value in Toric IOL Surgery”, the study was conducted by Kathryn Hatch, MD and the clinical faculty at Brown Alpert Medical School in Providence, RI. The study compared traditional toric IOL implantation on 27 eyes with toric IOL implantation on 37 eyes where ORA’s intraoperative aberrometer was used.
The article outlines various postoperative measurements taken in the study approximately two months after surgery, stating that 67% of the aberrometry group and 39% of the traditional group achieving “unaided” distance vision of 20/25 or better.
Dr. Hatch was quoted in the article as saying, “We conducted this study because to our knowledge, there is no report in the peer-reviewed literature evaluating the use of intraoperative aberrometry with toric IOLs. Use of intraoperative aberrometry to guide intraoperative decisions improves outcomes.”

Laugh More
I remember reading some years ago that children laugh somewhere around 300 – 400 times a day. In the same article it said that adults laugh as little as 15 times a day. Wow, that is a huge difference. To me it is sad statement that as we grow older we “lose” (or maybe we stifle) our ability to laugh. Me, I love to laugh. I don’t think that I am quite up to 300 laughs a day…but I get in about 100 a day. I believe that laughter is beneficial and extremely healing. I believe that it can be very therapeutic…not only for yourself, but those around you.
Dr. Richard Schulze said in an interview once that he encouraged his patients to learn 1,000 jokes as part of their healing process. While you might think this to be a tedious and unneccesary task to learn so many jokes, why not instead think of the laughter his patients got from taking on the assignment. While learning 1,000 jokes do you think their minds were focused on their illness or on laughter? I’m guessing that they were focused on laughing.
In his groundbreaking book, “Anatomy Of An Illness”, Norman Cousins describes how he started watching “The 3 Stooges” on a daily basis to “activate” his sense of humor and “regain” his laugh.

Editor’s Note: We often write pieces on the Center for Total Health blog about how important our surroundings can be to our health. We’ve covered passionate discussions around built environments, and we’ve highlighted success stories. What we haven’t been able to capture so far is the process. We have invited Scott Kratz, director of 11th Street Bridge Park – a newly launched project – to share with us some of his experiences as he takes this vision of a shared community space that supports health from concept to reality. This is his first post with us.
Can your zip code determine your health? Epidemiologists tell us that place – where we live and work – is one of the greatest factors in health outcomes. Urban planning decisions afect people’s health. We know there is a strong link between regular physical activity and lowered risk of obesity and chronic diseases. But what if there is no safe place to play? What if one’s home is located in a food desert, as it has been for many residents in Washington, DC, without access to healthy meal choices?
Linking place and health is a key goal of the 11th Street Bridge Park – an innovative project in the nation’s capital to transform an old freeway bridge into a new civic space. As the 11th Street bridges across the Anacostia River have reached the end of their lifespan, the DC Office of Planning and a local non-profit, Building Bridges Across the River, will use part of the remaining infrastructure to build the 11th Street Bridge Park, a new park above the river.
We have led an extensive public outreach campaign asking local residents for their programming suggestions and have received many inspired ideas and wonderful support. The community has suggested innovative play spaces, urban agriculture, an environmental education center and kayak / canoe launches on the river below.
Community outreach continues with a brainstorming session on Tuesday, March 25 at Kaiser Permanente’s Center for Total Health with presentations by 11th Street Bridge Park’s team. All of these ideas will inform a nationwide design competition launched in March to imagine the new park.
But we need to be more than aspirational. We need results. To understand the ways that access to green space, farmers markets, planting festivals and kayak launches may improve health outcomes, we are implementing a Health Impact Assessment. This baseline data of residential health will enable a comparative analysis after the 11th Street Bridge Park opens in 2017/2018. If we’re successful, we can make a solid link between health and place — and build a bridge to a healthier Washington, DC.

European tourists returning from Egypt with hepatitis A
Many European countries have reported an increase in hepatitis A cases, where travellers have been to Egypt. Many of the travellers had been to Sharm-el-Sheik or Hurghada in the south of the country.
Infection with the virus will result in a prolonged illness that affects liver function (so no alcohol for many months after infection).
The perception among travellers is that if they are staying in luxurious accommodation, particularly all-inclusive, then there is little risk. Hepatitis A is spread through poor kitchen or staff hygiene.
The disease has a very long incubation period (2-6 weeks), so vaccination can be taken on the day of travel and still be fully effective.

Baltimore Ophthalmologist Provides Premium IOLs That Reduce Or Eliminate Cataract Patients’ Dependency On Glasses
For cataract patients, Dr. Jay C. Grochmal offers a variety of Intraocular Lenses (IOLs) that can help patients attain clear vision after surgery without heavy reliance on corrective eyewear.
Baltimore, MD – Dr. Jay C. Grochmal, a Baltimore ophthalmologist, provides comprehensive eye care at his private practice, with procedures including LASIK, glaucoma treatment, and cataract surgery. With traditional cataract surgery, many patients have their natural lens replaced with a monofocal intraocular lens (IOL) to help them restore clear vision once the cataract has been removed. However, after correcting for near or far distance vision, the monofocal IOL may still require the patient to wear glasses after treatment. At Grochmal Eye Center, Dr. Grochmal offers a variety of advanced, multifocal IOLs as an alternative to monocular lenses, which can significantly reduce the patient’s dependence on eyeglasses or even eliminate the need for glasses altogether. These premium IOLs include:
- Alcon’s ReStor® multifocal IOL
- AMO’s Tecnis® multifocal IOL

While standard monocular IOL procedures are often covered by a patient’s health insurance, Dr. Grochmal says there are certain advantages that premium IOLs provide. He highlights that Alcon’s Toric IOL is very successful in correcting even high amounts of astigmatism (a common optical condition that causes blurry vision), and multifocal lenses in general are effective in significantly decreasing one’s dependence on glasses. He says many patients who receive multifocal lenses do not need to wear glasses at all after the procedure.
Grochmal Eye Center now uses the Zeiss IOL Master 500 for premium IOL procedures. The innovative technology of the Zeiss IOL Master 500 allows advanced calculations that measure the IOL power necessary to obtain optimal results for each patient. With this new technology, Dr. Grochmal is able to combine the results of the Zeiss IOL Master with his attention to surgical detail to produce distance vision improved to the point that his patients rarely require glasses afterward.
Multifocal IOLs can also be helpful for patients who do not suffer from cataracts but are looking to reduce their dependence on corrective eyewear. Patients who are not eligible for LASIK or other laser eye surgery procedures often qualify for premium IOLs, which have similar benefits. However, some patients who have already had monofocal IOL surgery may not be ideal candidates for the procedure. According to Dr. Grochmal, once a monofocal IOL is placed within the capsular bag of the eye, it is not safe to replace that IOL unless the exchange is done within a month of the initial surgery.
Dr. Grochmal stresses that multifocal IOLs are best used in healthy eyes that have no other eye diseases present, aside from cataracts.
